Vocational Rehabilitation for ESRD Patients QIA

A CMS project as of 2018, the goal of this quality improvement activity (QIA) is to increase the percentage of project facility patients, ages 18 through 54, who are referred to and receiving vocational rehabilitation (VR) services.

Reducing Blood Stream Infections (BSIs) QIA

Reducing Blood Stream icon

The goal of this project is to reduce BSIs using the CDC tools with a national goal by 2023, to reduce the national rate of blood stream infections in dialysis patients by 50% of the blood stream infections that occurred in 2016.

Increasing Transplant Waitlist QIA

Increasing Transplant icon

The goal of this project is to promote early referral to transplant and assist patients and providers to improve referral patterns by addressing barriers as patients move through transplant steps,with a national goal of 80% of new ESRD patients receiving dialysis at home or receiving a transplant by 2025.

Increasing Home Dialysis QIA

Increasing Home Dialysis icon

The goal of this project is to promote referral to home dialysis modalities, mitigate barriers to timely referral and determine steps to be taken to improve referral patterns goal to improve kidney health, with a national goal of 80% of new ESRD patients receiving dialysis at home or receiving a transplant by 2025.

Patient and Family Engagement

In compliance with CMS, the Network provides technical assistance to project facilities with a model of care that involves partnerships with patients/families in individualized care planning, support groups, and in facility QAPI programs.
