ESRD LMS Login

The ESRD Learning Management System is the hub for Network 16 dialysis facilities to complete CMS required quality improvement activities or projects. It is used to share important resources and to collect CMS required information. In LMS, you can:
  • Report data directly to the Network
  • Track your progress on CMS required activities (Facility Status Update Report)
  • Access a Resource Library
  • Use a team approach to complete requirements

The login information is provided to all facility administrators. This can be shared with any members of the staff that can assist in meeting the CMS goals and requirements. 

Username:  Your facility CCN.  

Password:  Contact your facility administrator for the password
